However, if "lac" is dictated for "laceration," or "dig" is dictated for "digitalis," "lac" and "dig" are what I type.

That is truly ridiculous. Incidentally, all the transcriptionists say my dictation is clear! Of course I don't usually have people with your skills, so I often submit punctuation, etc with the dictation. But the transcriptions are getting slowly worse with more blanks and outright mistakes. I never used to read them.

We're all getting weaker language skills, I agree.
