Jewel, it has been my experience that physicians and pharmacists routinely have a patient sign AND initial document(s) stating that they have been apprised of the instructions and contraindications and things to watch out for, as printed on that sheet of paper, and this is then filed with the patient's other information. " . . . corroborating evidence from credible professional experts that it was done . . ." Please define "credible professional experts." Thanks - Holly
