Jewel, I myself don't seek medical care very often. I am aware of what is generally done because I am involved in the documentation end of things.

That both the physician AND the pharmacist failed to document that this advice was given to the patient causes me to think that probably it was not.

Physicans and pharmacists, although they have had the benefit of higher education and have received certification/licensing in their particular fields is not synonymous with credible or ethical. Both stand to lose a goodly chunk and professional standing in the course of these events. Thus, they too have a healthy incentive to lie. - Holly
