The question is why wasn't this caught at test? ..Test hole.

This is an age old reliability issue which cannot be screened by an electrical test, a subject which was discussed ad nauseum on the Investors Hub a few weeks ago. Process control screens in wafer fab are needed, not more electrical testing.
