I assume under fog of war there is not a check and balance and perhaps no staff to perform such an operation.

Depending upon the command, it can be loose or tight. Some commanders believe in strewing them out.

For instance, the "rule of thumb" in a mission with an officer and two EM might be a Silver Star for the Officer, and a Bronze for the EM. Medal of Honor goes all the way to the top and is usually knocked down to a DSC or Navy Cross.
