Imagine the recklessness of someone with three kids who voluntarily cancels his family's health insurance to save money, then uses the savings to buy building materials to build a home addition with his own hands -- and presumably power tools, heavy carrying, working on a ladder from heights, etc.

Not quite as risky as Evel Knievel, but the potential reward ain't there, either.

One of the major factors contributing to shorter lifespans among the poor is smoking.

According to the Journal for Epidemiology and Community Health, people from lower socioeconomic groups are more likely to start smoking (especially as teens) and are less likely to quit smoking. Smoking is the single largest controllable factor for longevity (and health care costs as well). I don't think that choosing to start and then failing to quit a self-destructive habit is an issue of lack of access to preventative care.
