I think it's an error. It is so easily checkable that you would not lie about it.

Kerry is not the first SS with V found in a servicemember's records.

In his book, Stolen Valor, Burkett thoroughly documents every SS with V he came across. They were all phony.
None were clerical error.

Burkett's work was so good, it earned him a huge award from the Army.
