Do we know that the increase in risk is CAUSED by taking Viox and not by the absence of taking aspirin?

Now we do because we have a placebo controlled trial. Vioxx does substantially worse than placebo. A few months back we only had comparisons with Naprosyn, which was hard, because nobody really knows what effect Naprosyn has on cardiac risk. (An expert told me that he thinks that if you take it twice a day it acts like aspirin and cuts risk, but if you take it less often it doesn't).
