The volume of potential patients in the U.S. is small, about 5% of the potential patients. We are really talking about fluxuations in the luxury end of the market.

That luxury end (and to the extent that other countries pay high prices it might not just include the US) makes up a lot of the profit even if its only a small part of the volume.

So you still have a situation where "You can't have massive declines in prices and small declines in profits unless you have large decrease in costs or a large increase in volume, neither of which is likely.", unless you expect overseas prices to rise when American prices fall.
