"The killing was ugly!" Yes it was. But it is hard for me to blame the ones on the pointed end of the stick. Most of these are kids, usually stretched beyond what human beings can tolerate. Don't fall into the trap of blaming the ones at the lowest level. We know enough about how humans work to decide what the limits are for normal human beings. For something like the military, that can even be stretched, but even then there are limits. For what happened in Fallugah, those people were stretched beyond what you can expect humans to do. Not only was that Marine wounded severely just recently before that event, he probably didn't have much sleep. Both can impact judgement. If you want to blame someone/something, blame the system. You can't blame the individual when the system asks more that a human can reasonably deliver.
