I thought it was very clear. Time is limited, 24 hours in each day, 365 days in each year. Even assuming Bill Gates' time is "worth" more than your average obstetrician's, how to explain that he is now "worth" 40,000 (yes, forty thousand ) times what the obstetrician is?

Is he working that much harder? 40,000 X harder? Is he working "better"? 40,000 x better?

I think not.

What Bill Gates, Warren B, WMT and others do, which creates their wealth (and what the countless, nameless "little people"! do not do, the omission which fails to create their wealth), is master information. They are mastering information, accessing it, owning it when necessary, renting it when not, 3,000 x, or 10,000 x, or 40,000 x better than others.
