David, I see we are in agreement, except: cost-shifting and repricing DOES exist. Part of this can be traced to the insurance companies managing to whittle away at what the system bills their insured. There is nobody there to do this for the uninsured or self-pay patients. Therefore, they often are hit with substantially higher bills. Of course, there is always bankruptcy and many uninsured or underinsured who are hit with overwhelming medical bills do take that route. - Holly
