Both as a state employee, and as a self-employed person, I paid the standard percentage of my salary into Medicare. However, when you reach 65 you pay premiums into Medicare that are deducted from your SS.

It's pretty complicated. From SS inception until 1983 there was a clause that gave state and local governments the right to opt out, so often it's not the entire state. For example, in Texas three counties opted out before Congress closed the loophole. From my brief research via Google it appears that about 30% of the non-federal government employees are covered under an opt-out plan.
