From what I understand the fees of more than 2% are only charged by American Express, which is why so few merchants accept it - and why should they.

The other way to get charged more than 2% is by failing to qualify for a normal Visa Merchant account with a bank - ie merchants without assets and or a dubious reputation. They you must rely upon the tender mercies of resellers of Visa services who will let you clear your sales through their Visa merchant account for a premium price like 5% or 7%. The little website run by some guy out of his home might fall into this category if his bank won't do business with him.
