It was friendly fire. The car was mistaken for a suicide bomber car, as any civilian car that tried to run a checkpoint on that stretch of road would be. They shot into engine block to stop the car, and unfortunately killed one of the passengers.

That is the terrible effect of a war where one side's weapon of choice is civilian disguise and the car bomb against both soldiers and civilians. It's quite deliberate on the part of the terrorists, as killing civilians themselves, and making the Coalition kill civilians by mistake, are both successes for them.

Had the soldiers been trying to kill the passengers, btw, they would have used a machine gun, and there would be nothing left of the car or any of its passengers.

It's hardly a stretch to call this an "accident of war" or to note Sylvester seems to rejoice in every US mistake and setback.
