Couple more points:

Private insurers have huge overheads compared to Medicare, so we have huge resources devoted to controlling access to care instead of delivering care. Second, HMOs are constantly trying to improve efficiencies at all levels(except in the size of their bureaucracies!!), and technological innovations are making that possible to a large degree, but it takes time and $$. The simple law of supply and demand still applies-massive shortage of nurses, shortage of MDs in some specialties. But the biggest portion of the health care dollar, above all, is still........(drum roll)...drug costs.
