I doubt if Bush knew, but I would bet my shorts Cheney and Rumsfeld knew. For this type of thing to happen in such a pervasive way, it has to have been a culture of disdain for the lives of Iraqis that started at the top. As a lifelong corporate guy, I can tell you cultures start and end at the top. If a leader is rude, crass, and a workaholic, then that type of behavior becomes pervasive. If the boss is respectful of people and support work/life balance, then that becomes the norm.

Cheney and Rumsfeld knew. What's more, it wouldn't surprise me if they encouraged it. I'd bet my shorts on it.
