If you read Ms. Karpinsky's testimony, she was not allowed free access to all areas under her command all the time. I did not read this testimony very recently, but if I can remember the gist of it, there were operatives at the prison who were not soldiers under her and did not report to her. If my memory serves me, there were similar operatives at Guantanamo, and some part of the military command actually complained about them publicly because they were not following generally understood rules of treating prisoners.

Ms. Karpinsky seems to be saying that the rank and file soldiers who were posing the prisoners at Abu Ghraib were getting clear orders from someone.
