I take it to mean die/wafer. There was once a comment to that effect earlier in the 90nm ramp. Don't have a link

I keep hearing this, but has anyone stated for fact that this means percent of good die per wafer as opposed to numbers of good die per wafer?

The only meaningful measurement of yield is defect density and AMD isn't going to tell you that. This is the management team that, in one of the great snow jobs of all time, answered "line yield is @ 95%" when ask about yields by analysts. For those who don't know, this is a totally meaningless statistic compared to what the analysts were really asking and intended only to mislead. I wouldn't pay any attention to yield claims because you can't verify them from a fab that is selling well under it's rated capacity.
