You're basically talking about contracting out an expanded Medicaid. It's my understanding that Medicaid is pretty efficient. I don't know if it would be cheaper to do it privately and if it's not cheaper, what's the point.

What's not KISS about your proposal is that, beyond Medicaid, you have created the a program for the mandatory acquisition of medical insurance for the rest of us, which means you need regulations and bureaucracy and an enforcement mechanism. Lotsa overhead there. If you don't enforce it, you'd still have people opting out, then presenting themselves at the emergency room unable or unwilling to pay. Which is just what we have now. You can't not service them, or so you say, or make them pay. So what you have is the same as what we have now. So where has this big new initiative gotten you?
