1 - Note at the end of what you quote from the site. "One Bush official called the accounts of both incidents 'flatly untrue.'"

2 - Libya - If this is true it only means that some British diplomats don't like Bolton, or didn't like his position on Libya and that he was kept out of the loop. It would mean he should get no credit for the apparent success there but it doesn't amount to him doing anything worthy of blame.

3 - Iran - re: "Bolton is being too tough." - Probably a point in his favor.
