In the hood they sell "loosies" from an open pack although the practice is illegal. As a kid, i remember them selling loosies at the neighborhood candy store. Must have been for 2 or 3 cents a piece given that a pack was 25c.

In the movie "Dead Again" (with Kenneth Branagh and Emma Thompson) Andy Garcia's character smoked a cigarette through the hole for his tracheotomy (sp). Now that was creepy.
