This raises a queston I have been pondering for years. Why not stratified health insurance? If people want health care for $100 a month, give them a plan they can afford. Many expensive procedures will not be available to them. It is likely that some drugs will be unavailable to them. If someone is willing to pay $2000 per month, shouldn't they be able to buy a much better quality plan that would prioritize them in queue, and provided greater availability of treatments?

($100 and $2000 are arbitrary.)
