Actually, he's dead. He caught the Instant Cancer (perfected for use with Jack Ruby and others who were an embarrassment) while in prison. After being beaten up several times by fellow inmates. I still remember the scene where Rod Steiger gets the crap kicked out of him at Alcatraz and runs cowering into a corner screaming, "but I'm Al Capone."
