Once you have the nuke, put it on a ship or large boat, sail in to any port and blow it up. That isn't very hard.

The hard part is getting the nuke. And for some enemies there is a lot of deterrence and very little motive. But some of our enemies aren't as easily deterred, and are prone to act in ways that from our perspective are irrational. I'm fairly certain that no terrorist group has a nuke, but if they do manage to get one I'm not so sure we can stop them from using it. The main way to stop them is to have good intel on them or on the nuke and its location, and to seize or dismantle or destroy it before it can be used. The only real big if is getting the bomb, of course that one is real big, but perhaps not insurmountable.
