That is a broad generalization and unfair. Do you realize how many entries, raids etc are conducted every day without incident.

If you are going to run a railroad, you are eventually going to have a wreck. There are dozens of ways that could have been screwed up. It is a very small percentage that end up like the one in the article.

You can bet your bottom dollar the cop that made the error is in the hot seat and will be disciplined.
