LB, The leadership of those countries is the main problem, as you have mentioned. Any foreign aid sent to one of those countries goes straight into the dictator's pocket, and the people that need the aid so desperately, are left in the dust.

Limited, targeted, aid sounds like a good beginning. For the people there, a way to reach them personally has to be found. If the UN was worth a damn, they could oversee a project, but there seems to be as much corruption in the UN as in any dictatorship.
