I can't believe the sharpshooter(s) blew it. I know he shot at police first, but you don't unload a bunch of sidearms at someone holding a hostage. I hate to say it, but I think they blew it.

accidently shooting the baby while trying to bring this nut case down in order to save themselves.

I suspect that the internal review of this case will fault the swat team for going in instead of waiting the man out. Of course, if he had killed the baby while they waited they would have been faulted. Tough decision.
