The Fed may hope that the category of Euro Dollars includes all offshore U.S. Dollar deposits.

But there is a gigantic leak somewhere in their counting system and this is the most obvious source of the problem.

Keep in mind that the "Money Supply" is in reality most nearly the total amount of credit extended, in other words the debt supply. You are not really trying to trace currency flows, but extensions of credit - even from one commercial firm to another.
