Well, I'm not so sure that is always true. Permanent bans for the likes of American Spitbot, whackode00, Toiletpaperpaw and that ilk are no reflection on the moderator....more simply they are just good thread hygiene...

I don't know. After 4 or 5 temporary bans, perhaps it's time
to make it permanent.

I have several on my thread that keep doing the same thing
over & over. If they do it again, it may just be for the last time.
