I was thinking mostly of doctors of education when I wrote that post. My father was an educator, and did everything for his doctorate except finish his dissertation (he left the field while he was writing it, so it became less essential). He and his friends used to laugh at the people who insisted on being called "doctor." When I was in college, "professor" was the only term of respect used, actually. But it was so informal back during the hippie days first names were sometimes what the professors liked to be called.
