Human to human is easier because those taking care of sick relatives don't realize the dangers and therefore get infected, probably from vomit, feces, or saliva.

Bird to human is very rare, but that may be changing in Indonesia, where the source is unknown. However, infection of office workers suggests some unusual exposure, or those are the hardest to cover-up, so they represent the tip of a large iceberg represent the many who come into daily contact with infected birds or pigs, but don't get tested and die undiagnosed.
