a private system should be available, that recieves no public funding. I believe England has a system such as that. The private system is often accused of sabotaging the public system (hiring away key personel with unusually high offers, dropping patients that are chronicly sick, and other tricks). This creates a crisis in the public system, which is exasperated by vocal complaining paid for by the private system, all to get more people to turn to the private insurance and practice.
