I am sure it has happened more than once, and I am sure there are many unreported instances, where the miner did not survive to make the report.

The incident that stands out in my mind, was the disaster at the coal mine in Springhill, Nova Scotia. A "bump" occured, caused when mining of the coal seam removed pressure from the floor of the seam, and a section of the floor violently up-lifted creating an obstruction. Sparking from rock on rock caused a methane gas explosion, and 90 some men were killed. Several were trapped without food or water, in bad air and survived until weeks later when rescuers reached them.

Glad to hear you are so bullish on moly, Elizabeth. I am not so sure. I was sure before I acquired a large position in UNB, and fear colored my thinking.
