hospitals get paid a "case rate" for a diagnosis...fee for service is getting much more rare for hospitals..
so if patient was admitted with nausea and vomitting with dehydration post chemo then they would get a fixed rate for that diagnosis..
best for them to use zofran over aloxi if efficacy is only slightly inferior but costs much less...

thats why you also see substitution by hospital pharmacies by fairly equivalent ace inhibitors or lipitor/zocor/crestor
