I was actually just filling out the possible boxes in my head, not thinking of an illustration. However, on your first point, while professional organizations do have some influence, they don't have determinative influence. There can be quite a bit of slippage.

As for a different illustration, perhaps, back, I hate to say it, but it is in my mind, to the abortion discussion. Many states have passed new laws dealing with notification of parents. I expect that abortion providers are more than graphically aware of the incidence of parental sexual abuse leading to pregnancy leading to a child asking for an abortion without the parents knowing anything. I would guess that abortion providers experience some very severe ethical conflicts in such situations. Both because of the not knowing the truth of the situation and of knowing the incidence of such.
