I waited tables for a long time- but it was before the reporting requirement (or maybe the law changed toward the end of my waitressing career- after all, I was in college for much of that time. I can't say I remember the tax angle all that clearly for those years). I'd assume higher end restaurants are different- because they are adding on 15-20% routinely, and I'm sure they don't want to get audited. I'd be happy to call one of the better restaurants around here and ask, if you are interested.
