Alberta used to do this, late 70s / early 80s IIRC. Once or twice a year you got a record of all your interactions with the system, and what the doc charged the AHCIP for that interaction. For most people, it was an effective means by which to be reminded that health care isn't "free." I think it was killed because a minority thought they were being invoiced, and couldn't be convinced not to bitch as a consequence.

printout of what health care each patient received privacy advocates/activists will hate this, but there are abusers of the system and there should be a system of checks and means to flag and stop this stuff. Abusers simply load up the system which in turn makes the waiting lists longer.
A guy that worked for one of my companies for a short time, on payday would take his kid to emergency and check him in with a cold/flu or some such thing so he and his shack-up could go partying and not have to waste any good beer money on a babysitter
