I found it!!! It was in a pile of magazines. So much for being organized. It's a 1099B - Proceeds from Broker and Barter Exchange Transactions. It's for $22.66 which I assume was the final sale price per share when the team was sold.

It's not like they sent me a check or anything. I assume that's just the value they're assigning to the cert that I never sent in? I guess I have to claim it somehow

Ha! Just found this. $22.60 my behind! On sale for $199.95.
