It is like the FICA tax. It comes off of your check and you never really think about it. Even taking that deduction and allowing redirection into a HSA where the employee feels some ownership would help.

Eliminating the third party payer system would help. As Brendon Miniter said, Health Insurance companies seldom communicate in any way but to tell the beneficiary how much they paid. The pre authorization requirement some policies require for some procedures is not really an effective means of communicating.

Doctors are asked only if they accept XYZ insurance, not what they charge for a procedure. If you need a procedure and it is going to deplete your asset (HSA balance) you are likely to be more interested.
