Clearly, EVERY NPT participant country was supposed to "undertake effective measures in the direction of nuclear disarmament". It is also evident that there are a number of countries that didn't take that part of the treaty very seriously or thought that it didn't apply to them. Once that happens, it only reinforces the determination of others to acquire the weapons themselves and therefore over time render the treaty useless.
