If the rates where more transparent I think there would be downward pressure on them even despite the insurance money sloshing in to the system and the fact that people are for obvious reasons reluctant to walk away from medical care if they can't get a deal.

That factor might not be enough, but then you could have medical care buying services that negotiate the rates the same way insurance companies do just without paying for the care. Perhaps this would be combined with catastrophic insurance.
