I think it has something to do with the puck crossing the blue line with nobody around. Or with fan leaning out and catching a pop-up. You need to have 2 opposing players between you and the goal when the play starts, unless the ball is ahead of you. When the ball is kicked, you can break by the defender, but you can't just sneak down behind them and wait for a pass. More or less.
