Conduct is not policy. Policy is guide to action and a measure of conduct.

It is possible to have a formally stated policy and an incompatible informally endorsed policy. I can't tell if that is what you are trying to address or not.

If a school has a policy that teachers can't smoke in the teacher's room, and then a teacher lights up a cigarette in the teachers room (which is against policy), did the school's smoking policy change? Or was it just violated?
