I don't see how special endorsements would compromise the program if they had to be achieved with rigorous validation presented at the DMV, and if there were fines in place for abuse. People don't park in handicapped spaces because the fines are just too big- I think the same sort of program for people impaired by seat belts would be no problem at all- especially since it would probably involved a fairly small number of people- many fewer people than the handicapped program, for example.
