Continuous communications might be going on, but not directly between everyone involved. And even if they where in direct radio communication its not like they where telepathically linked. If I look at something for 75 minutes I am more likely to understand what it is, than if I am one of dozens of people who got a look at it for a few seconds to a few minutes, mostly at a distance, and who relayed impressions over the radio to third parties, who talked to other third parties, who talked to me. Friendly fire incidents, even protracted friendly fire incidents are nothing new.
