Number two is far more believable, and to an extent may be true, but defense against an unexpected attack is rarely very good. It doesn't take any unusual level of incompetence to fail, just your ordinary everyday normal level of incompetence or mistakes. It doesn't even always require that. Systems that are very competent can fail when they have to deal with unexpected situations.
