But that is a PPO as i understand it. Dr. bills for service at retail and plan pays wholesale based on docs/clinics relationship with ins co. Doctors are now charging for services plus malprac insurance as well as bookkeeping costs. I dont put the blame there. I am from a family of doctors whose kids are passing on medicine because of their dads/moms disillusionment with the current system. There is a body of thought out there that believes that docs should be on salary on per hour basis to better serve patients and rationalize their practices.

Some providers won't accept Medicare because they consider the payout from Medicare to be too low and they aren't allowed to accept the Medicare payment and charge the recipient extra on top of the Medicare amount.
