but I've never actually used a credit card to get cash that way

I have no bank account so all my cash comes from sticking my Amex card in somebody's ATM. My Amex card, whether I charge a purchase or use an ATM, draws on my Fido acct. It's all transparent to me except for the part where the ATM asks me if I will accept the charge. Neither Amex nor Fido charges me but the bank with the ATM does.

I take lots of cash each transaction and spend it over many months so I consider the ATM charge a bargain.
