With a public system there would still be horror stories, waiting for care, denial of expensive care etc.

As for other nations spending less - A number of the reasons why we pay so much are summed up here

In addition to that, American medical education costs more and American doctors charge more. While its possible that some federal program could greatly cut pay for doctors, its more likely that that cost will continue, and would just be paid by the government, or by the government and private companies if we don't go to a true "single payer" system.

I've already addressed the point about administrative costs. A lot of the overhead for government provided health care isn't counted in the figures for their costs.
